Verify that the correct patient chart, or eMAR, is accessed in order to write the verbal order. Also, note any special instructions.

In addition, note that this was an oral or telephone order and sign as the nurse who took the order. Many facilities require the HCP to sign the order within 24 hours. Some require designated personnel to countersign medication orders written by HCPs, who are not physicians.
